Payment Plan Information

Agapē offers a standard payment plan for patients who have income less than 200% of the Federal Poverty Level (see image below). Approval for the payment plan is determined by an income verification process. The payment plan works as follows:

• 50% of the cost of the service will be charged at the time of service (for self-pay individuals) or at the time Agapē receives remittance advice from a patient’s insurance company.

• The remaining balance will be added to a tab that patients can pay down over time when their financial situation improves.

• Other ways that patients can reduce the cost of therapy include:

• Asking their counselor to reduce the frequency of therapy. For example, from once a week to every other week.

• Asking their counselor to reduce session time. For example, from 60-minute sessions to 45-minute sessions, or 45-minute sessions to 30-minute sessions.
